**YOU WILL NEED TO FILL OUT A REGISTRATION FORM FOR CHILD YOU ARE REGISTERING.

Religious School Registration Form

The term affiliation means you have a registered membership and are

current in your financial obligation with either Temple Beth Tikvah, Congregation Shalom Bayit or Chabad.

Does your child have any allergies, medical issues or food restrictions?
Is there anything you want your child's teacher to know about them or their learning style?
I give my permission to the School to use my information in a School Directory that will be distributed to School families and staff.
As a small Religous School, we ask all of our families to volunteer for at least one "team" during the school year. If you are unable to volunteer, you may make a donation to the school instead. Please choose all that apply below.

By registering my child and signing below, I understand that I am expected to join at least one "team" above and/or make a donation to the school, as well as volunteer for one Religious School session per child enrolled (help with set-up/clean-up, assist teachers when needed, monitor visitors during school hours) during the school year. I will mail a check payable to Jewish Community Religious School for the school registration fee upon completion of this form to PO Box 7472, Bend, OR 97708.
